Job Description:
The Ummah Futures International LLC is hiring a health professional to address violence in Miami-Dade County. Jackson Memorial Hospital is home to the Ryder Trauma Center and health professional will be engaged in collaborative partnerships to increase the wellbeing of clients and families in Miami-Dade.
As the health professional for the Hospital-based Violence Intervention Program (HVIP), you will be working in collaboration with Jackson Memorial Health System Trauma Division. You must be a State of Florida licensed health professional. You will coordinate with other members of the team to provide community services for survivors of gun violence after discharge from the hospital.
As part of Miami-Dade County’s Anti-Violence Initiative (AVI), which is designed to reduce group violence. The AVI consists of four primary components: (1) Prevention, (2) Intervention, (3) Suppression [Group Violence Intervention (GVI)], and (4) Reentry. The services provided by the health professional fall under all four primary components.
DUTIES:
· Provide compassionate personalized nursing care to patients that have been victims of gun violence in the Miami Dade District.
· Assess and evaluate the physical and emotional needs of the patient that affected by gun violence.
· Collaborate with the multi-disciplinary team of the violence intervention program to formulate a cohesive patient plan of care -this team includes case managers, social worker, violence prevention coordinator and medical team with the discharge plan of care.
· Able to work with patients that are victims of gun violence with an understanding of the patients’ needs in an under service community.
· Able to provide wound care service in the nursing scope. Specializing in assessing wounds, treating patients with complex wounds, ostomy or continence condition.
· Administer treatment plan according to the discharge instructions. This includes intravenous, intramuscular, oral and topical medications.
· Able to work with other team members within the program to provide personalized care for wound treatment and infection prevention with the goal of prevention readmission to the health care facility.
· Able to take care of common types of complex patient’s wounds including open wound after gun violence and resulting complications which includes but not limited to ulcers, pressure injuries, ostomy, abdominal incisions, surgical wounds, appropriate wound care and prevention of surgical site complications and infection prevention.
· Working in collaboration with the discharge multi- disciplinary team from the acute health care institutions to determine at home services needed for the gun violence victim after discharge.
· Educating patients and caregivers on prevention measures and home care treatment plan including but not limited to infection prevention, pressure injury invention, wound care and key risk factor to report.
· Assist in advocating for the wellbeing of the patient that has been a victim of gun violence in the underdeveloped, underserved communities in the Miami Dade district.
